  Reminder:  6th graders must have a complete physical that is performed on or after July 1, 2007 (blue State of Connecticut form.) This should be on file in the nurse's office by February 15, 2008. If you have not yet turned in your child's physical, please send it to the nurse by the above date. If you are unable to turn in the physical by February 15th, please contact Mrs. Shea @ 264-2767.Thank you.
 

 ** Scoliosis screnning will be done for  all students that have not had a physical this school year during Physical Education classes in early March** Girls should bring a bathing suit to class. (The actual date will be announced when it is set.)

            HEALTH OFFICE:  IMPORTANT INFORMATION ON TUBERCULOSIS POLICY

 
To be in compliance with State of Connecticut Regulations, Region 15 adopted a policy on Tuberculosis in February 2006.
 
  1.  Students entering the Region from outside of the United States must have a TB screening with the results  interpreted and documented by their physician's office PRIOR to entering Region 15.
 
 2.  Additonal TB screening may be  required by the school medical advisor and /or school nurse under the following circumstances:
      a.   if a student has traveled to a high risk country, stayed for at least a week with substantial
            contact with a high risk population,
      b.  if a student has had substantial contact with someone from a high risk country,
      c.   if a student has had contact with someone suspected to have TB,
      d.   if a student has had contact with  anyone who has been in a homeless shelter, jail,
             prison, uses illegal drugs, or has HIV infection.
 
If you will be traveling on vacation to any of the areas listed below, this policy must be adhered to.  Please contact  the  Health Office with any questions.  This will require 2-3 additional absent days for your child/children.
 
 
High Risk Countries noted by State:  All countries in Africa, Asia (including former Soviet Union), Eastern Europe, Central and South America, Dominican Republic and Haiti.

Allergy Alert: We have a number of students with peanut allergies. Please keep this in mind when sending in snacks that may contain peanuts. We would like to minimize any potential exposures that may negatively affect students. Thank you.

  • Gym Excuses - Please give all doctor's excuses for injuries and restrictions to the Nurse.
  • Vacations - Please make sure all notes re: vacations go to the nurse's office.

Reminder: Physicals are needed yearly if your student is trying out for a sport (they are good for THIRTEEN MONTHS from the date of the physical. - See Link Above) Thank you.

Please remember to call your student absent or tardy to the guidance office at 264-2685 or the nurse’s office at 264-2767 before 9 a.m.

Remember, students must be home for at least 24 hours after fever, diarrhea or vomiting symptoms cease. Make sure students are well enough to participate in a full day of school.

Field Trips- Any student in need of a medication on a field trip MUST notify the nurse’s office 2 weeks prior to the field trip. A doctor’s order is mandatory for prescription or over the counter medications. Also notify the nurse’s office of any medical conditions that students may have (even if we are already aware of it.)

** PLEASE NOTE: Parents are responsible to bring in the child's medication for field trips from a home supply. Medications from school WILL NOT be used.**

Sports -  Physicals are valid for 13 months from the date of the physical. Please check the nurse's office to make sure your student’s physical is on file. Forms are available in the main office or nurse’s office. * No student may tryout without a valid physical.
